Uncertified copies are no longer issued under current Ohio law. Mail requests require the completed vital records application form, a self-addressed stamped envelope, and a check or money order payable to Morrow County Health Department. Processing takes approximately three weeks. Social Security numbers are redacted from death certificates unless the requester completes the Proof of Relationship Verification Addendum per Ohio Revised Code 3705.23.
